What should I do if I don’t have my 9-Digit Unique ID Number?

PFRPA members, who do not have their 9-Digit Unique ID Number, should email PFRPA at [email protected] before filling out their Early Enrollment Form. Once you receive your Unique ID, then return to fill out your Early Enrollment Form.

I previously filled out an Affidavit for Spousal Eligibility to add my lawful spouse to my dental and/or vision coverage. Does my spouse also need to complete an Early Enrollment Form?

Only the enrolled PFRPA member needs to complete the Early Enrollment Form. This will ensure uninterrupted coverage for both the member and their spouse for the 2027 plan year.

What happens if I do not complete Early Enrollment?

PFRPA will continue to offer rolling enrollment, so you will have access to the PFRPA Dental and/or Vision Plans when you need them. You will just need to contact PFRPA before your visit to the dentist or eye doctor during the next plan year.

Rolling enrollment requires additional steps and time to process. We recommend participating in the Early Enrollment process each year to ensure there is no interruption to your coverage from one year to the next.

When does the Early Enrollment period start and end?

Early Enrollment for the 2027 plan year will run from October 1, 2026 to December 15, 2026.
